Non-invasive weaning from ventilation reduces mortality, ventilator-associated pneumonia, and length of stay in intubated adults.

چکیده
Objective: To review the evidence as to whether early extubation with immediate application of non-invasive ventilation reduces mortality and ventilator-associated pneumonia and improves other outcomes in criticallyill adults receiving invasive ventilation. Data sources: Medline, Embase, CENTRAL, searched up to April, 2008. This search was supplemented by hand-searching of conference proceeding and citation tracking. Study selection: Randomised and quasi-randomised controlled trials involving adults with respiratory failure who required invasive ventilation for at least 24 hours in which extubation with immediate application of noninvasive ventilation was compared to continued invasive weaning. Outcome measures were mortality, ventilatorassociated pneumonia, weaning failure, length of stay in intensive care or hospital, total duration of ventilation (invasive and non-invasive), duration of ventilation related to weaning (after randomisation), duration of invasiveNon-invasive weaning from ventilation reduces mortality, ventilator-associated pneumonia, and length of stay in intubated adults

Use of non-invasive ventilation to wean critically ill adults off invasive ventilation: meta-analysis and systematic review
OBJECTIVE To summarise the evidence for early extubation with immediate application of non-invasive ventilation compared with continued invasive weaning on important outcomes in intubated adults with respiratory failure. DESIGN Systematic review and meta-analysis of randomised and quasi-randomised controlled trials. SETTING Intensive care units. PARTICIPANTS Critically ill adults receivin...
متن کاملNoninvasive ventilation as a weaning strategy for mechanical ventilation in adults with respiratory failure: a Cochrane systematic review.
BACKGROUND Noninvasive ventilation has been studied as a means of reducing complications among patients being weaned from invasive mechanical ventilation. We sought to summarize evidence comparing noninvasive and invasive weaning and their effects on mortality. METHODS We identified relevant randomized and quasirandomized trials through searches of databases, conference proceedings and grey l...
متن کاملWeaning from ventilator and effect of Blender-Humidifier on outcome of it
Background and objectives: the weaning procedure of mechanical ventilation in many patients is a difficult and long process and increases the time of mechanical ventilation. There are numerous ways to achieve this goal. One common way is using CPAP-ventilator. Considering the lower price of Blender-Humidifier compared to CPAPof ventilator and the limited number of studies in this field, this st...
متن کاملReview Paper: The Bed Incline and Prevention of Ventilator-Associated Pneumonia
Background: Ventilator-Associated Pneumonia (VAP) is the most common nosocomial (hospital acquired) infection among patients undergoing mechanical ventilation. It increases mortality rate, duration of mechanical intubated ventilation, and hospitalization in the Intensive Care Units (ICUs). This review study aims to determine the proper gradient of a hospital bed in preventing VAP in patients ho...
متن کاملThe effect of presenting complaint on the risk of developing ventilator-associated pneumonia for patients intubated in an academic emergency department
Introduction Ventilator associated pneumonia (VAP) is a complication of mechanical ventilation that increased ICU length of stay and mortality. Eckert found 26% of trauma patients intubated in the ED develop VAP as compared to 6.5% of those intubated in the ICU. Green demonstrated that 70% of critically ill patients were intubated pre-hospital or in the ED and 18.2% remained in the ED for more ...
